How to overcome from 250K+/Month + to 55K+/Month LKR downgrade pay scale ?

Middle of Next year ,I have no choice but to accept as an “24 hour intern medical officer position” in Sri Lanka.I have no intention to work in a private sector after 1 year of internship & take out the money from the patients pockets.I do solo business consultancy service in international level ,but unfortunately I have to stop everything in a few months.Even after successful completion of internship, want to do only government practice & do business in free time.I am passionate of doing medicine but not as a job for living.Need serious advices from those who are matured and experienced.
