Pending Homelessness and Don't Know What To Do

I live in Michigan and my current lease for my apartment ends on May 19th. I had plans to move into a new place next week so that this wouldn't be a problem, but I keep getting in my own way and getting myself fired from whatever job I get (usually because of oversleeping), so now moving there is no longer an option as I don't have the money needed to do so. I don't know what to do, I've never been this scared or lost in my life and I really don't know what to do or where to start with not having a roof next week. I've been in contact with every resource I could find online, and with my state's unemployment, but I got turned away or denied from everything. Any advice on what to do or how to prepare would be greatly appreciated.
