What's up with the sandra dorsett quest?

I got call from her to deliver a data shard to her at her apartment and apparently I already had it since she asks me whether I had it or not.

I went to her apartment to deliver the data shard and the data shard is already cracked by me which I definitely didn't do.

I have no idea how I got the data shard in the first place I didn't go to whatever scav place she was mentioning.
