What does it mean by “PDF form submission” on common app? How do I get the PDF for my teachers?

I need two letters of recommendation for most of my colleges, and the second one is requiring me to submit a PDF? It’s telling me to download the PDF from my school and then give it to my teachers so they can fill it out but I can’t find anything on the subject. Can someone please help with this?
